Transitionary Services 

Seven Care Services aims to bridge the gap between children and adult care provision. We understand this can be an uncertain time however, we have a wealth of experience supporting people and their families through the changes that transition involves. 
Our services for young people include supported housing services which provide a safe place for young people to live, learn and make the transition to independent adulthood. 
 
We also offer move-on services to help residents find and maintain their own independent accommodation, as well as floating support or community outreach services where we provide one-to-one or group support to young people in the community. 
 
We are open and transparent throughout the whole process and understand there needs to be clear lines of communication. 
Service Provision: 
 
Providing a nominated case manager to co-ordinate the transition process. 
Signposting to and navigating through information. 
Assistance in evaluating available options including accommodation. 
Undertaking a holistic assessment, to include a transition and person-centred care plan. 
Matching support teams that include education, therapeutic and clinical staff as required. 
Involving family and circles of support 
Providing support in all locations; including home, college, place of work, or in their local community.
