Complex Community Care 

Seven Care Services provide bespoke, domiciliary care to children and adults with a range of support needs. 
Our tailored home care services allow you to remain independent in the comfort of your own home, without compromising on the quality of your care. 
 
Our service is overseen by a clinical lead and regulated by the Care Quality Commission. All our Care Assistant Practitioners receive face to face training and have competencies assessed. They receive regular supervision and are provided with 24-hour support. 
We assist children, adults and their families/carers with: 
 
respiratory support that includes oxygen therapy, tracheostomy care management and ventilators; 
nutritional care for example gastrostomy and nasogastric tube management; 
administering various medications; 
general care to meet personal hygiene needs and maintain dignity; 
epilepsy management; 
supporting individuals with neurological, brain injuries, spinal cord injuries and auto immune conditions; 
supporting mental health conditions such as dementia and challenging behaviour associated with learning disabilities. 
Support Settings 
 
24 Hour Live-in Care within the family home. 
Educational support within a university or college. 
Independent living/supported living scheme. 
In-reach/one-to-one support in residential home or hospital.

Initial referral/pre assessment 
Formulation of bespoke person-centred care plan 
Assigned clinal lead and introduction of care assistant practitioners. 
Bespoke training provided to staff to support your individual needs. 
Regular review of service 
Communicate with partner agencies such as education and health.
