Thank you for completing our online form to begin the process for an Autism Assessment through the Right to Choose pathway. 

Please notify your GP that you wish to use Seven Care Services for your Right to Choose, and we will need a referral letter from them to initiate the process. If you have provided the GP email address, we will send them a request with our referral template for their convenience. If they do not have a public email address but do require our template, please ask them to contact us directly. 

 

Upon receiving the referral, we will be in touch in due course with our information gathering forms. 

Please note you will only be added onto our wait list once we have received the GP referral.
